In applying to compete in Tag Team Surfing Titles I acknowledge that there are inherent dangers associated with the sport of tag team surfing and serious accidents can and do happen which may result in me being injured due to my participation in the Tag Team Surfing Titles.

I confirm that I am suffering no illness or health issues that may put me or my fellow competitors at risk due to my participation at the Tag Team Surfing Titles. To the extent permitted by law I agree to absolve and indemnify the Carter Surfing Industry and its sponsors, respective councils, its directors, officers and employees from and against any and all liability for injury, loss or damage however caused out of my participation in the Tag Team Surfing Titles.

I agree to release and forever discharge the Carter Surfing Industry and its associated sponsors, respective councils from all claims that l may have had or may have had but for this release arising from my participation in the Tag Team Surfing Titles. I agree that the Carter Surfing Industry retain the right to utilize images of my participation in the Tag Team Surfing Titles for the purposes of promoting Tag Team Surfing and Carter Surfing Industry.

I have read, understood, acknowledged and agree to the above declaration including the warning, release and indemnity. I affirm that l have not altered the official competition registration form in any way. The Tag Team Surfing organizers reserve the right to change/cancel events, times and duration of match times and rules due to surf conditions.
